It would only be appropriate to refer to Johnny in the past tense in relationship to Casey. It is such ancient, brief (and small) history.

Yes. In fact they were still dating when she did Road Rules: Viewers Revenge which was in 2007. They are no longer together because he allegedly cheated on her.
